I like to tell my patients that the body functions like a finely tuned kinetic chain. If one link in that chain is broken or rusty, the whole system shudders.
Think about how you walk. Your foot hits the ground, your ankle absorbs the shock, your knee flexes, and your hip rotates. It is a symphony of motion. But what happens if your lower back—specifically the lumbar spine—is misaligned? That misalignment, or subluxation, forces your hip to compensate. When your hip compensates, your femur (thigh bone) rotates internally, and suddenly your knee is twisting in a way it wasn’t designed to twist.
Years of this “compensation” lead to wear and tear. You might feel the pain in your knee, but the culprit is often hiding in your lower back or pelvis. This is why I get so passionate about proper diagnosis. Treating the knee alone in that scenario is like putting a bucket under a leaking roof but never fixing the shingles. You have to find the source.
Understanding Hip Pain
The hip joint is a ball-and-socket marvel, designed to withstand a tremendous amount of force. However, it is closely tied to the sacrum and the ilium (the large pelvic bones). When I am caring for a patient with hip pain, I am looking for a few specific things:
- Sciatica: This is a big one. The sciatic nerve runs right through the hip and down the leg. If the vertebrae in the lower back are subluxated, they can pinch this nerve, sending shooting pain through the hip.
- Sacroiliac (SI) Joint Dysfunction: The SI joints connect your spine to your pelvis. If these get “stuck” or fixated, it causes inflammation that mimics hip joint pain perfectly.
- Osteoarthritis: While we can’t reverse aging, chiropractic care helps mobilize the joint, ensuring that the fluid inside (synovial fluid) keeps lubricating the cartilage. Motion is lotion, folks.
I have seen patients who were convinced they needed surgery find relief simply because we restored proper motion to the pelvic girdle. It is not magic; it is biomechanics.
The “Middle Child” Syndrome: Your Knees
The knee is often the victim of the joints above and below it. It is a hinge joint—it mostly just wants to bend and straighten. It does not like to twist. But if your hips are tight or your ankles are collapsed, the knee gets caught in the middle.
Chiropractic care for the knee involves ensuring the patella (kneecap) is tracking correctly and that the tibia and fibula (shin bones) are properly aligned with the femur. We also look at the surrounding soft tissues. Often, a tight iliotibial (IT) band pulling on the knee is actually a hip issue in disguise.
By adjusting the spine and the extremities, we take the torque off the knee. It allows the inflammation to subside naturally because the joint isn’t grinding against itself with every step you take.
